Registered certification candidates must use this form to schedule their examination by at least 5 business days prior to their desired examination day.

The online exam may be scheduled for any day and taken from anywhere with any Internet connected device except on listed dates below. Candidates can schedule an exam up to 3 times in total without additional charges.

The online exam will be distributed by email and will remain active on the scheduled exam day.

After you submit this form, we will assess your eligibility to take the exam and send you a confirmation email by the next business day.
